I do find that the neck of the 21mm is much shorter than the 24mm - it doesn't cause any problems for me, but I'm sure it would for some people. I just wish that Medela made 19mm! I'm contemplating getting non-medela shields from Amazon as they carry 19mm.

Rated 3 out of 5 by from Too big! I'm preparing for having my son and I have all of the Medela shields available to purchase. The smallest one is still going to suck my areolas halfway down the tube of the flange. How am I supposed to pump if the pump is going to eat me?! You guys have got to get on the ball and make smaller flanges. They need to start at 15mm maybe smaller and go up from there. Until you offer official Medela shields in small enough sizes I'm going to be forced to get the unofficial smaller flanges that a different company offers and are sold on a major hub site. I'm worried about how good they will be because they aren't a name that I know and trust but you guys have dropped the ball for breastfeeding mothers!

Please don’t write your “we are taking into consideration for future...” Tell us exactly when you going to have smaller size shields and is there a different company that makes shields that would fit your pump!

Medela is such a large company and I can't believe smaller shield sizes have not yet been made avaialbe after so many moms have addressed this same issue. Please do something about this to help the moms who are using your pumps and are stuggling!!

I've tried multiple off brand shields but none of which fit the Freestyle and it's so frustrating! I think this problem is a LOT more prominent than realized... please, please, please make smaller shields asap! I would love to continue using my Medela pump but clearly that's not going to happen until smaller shields are offered

I had to have emergency surgery after my sons birth, and due to my small nipples and groggy state, I had to use nipple shields to encourage breastfeeding in the first few weeks of my sons life. I soon developed De Quervain's tenosynovitis in both of my wrists, which made maneuvering my son into latch position extremely painful. Pumping became the only way that I could continue feeding my son breast milk, which is extremely important to me. My husband bought me the Medela Free Style, thinking it was the best of the best. It was a non refundable purchas for obvious reasons, so we put a lot of stock into your reputation. When I realized that the standard flanges didn't fit, I purchased the 21mm adapters at additional cost. They are still too big! Unfortunately I continue to struggle, as this extremely expensive pump does not come with flanges that are small enough for a proper fit. As a consequence, I have suffered from plugged ducts on numerous occasions and pumping takes me a very long time. No other companies make flanges in a smaller size that are compatible with this model of pump, and I am not in a position to spend another $600 on a pump that has a flange size that will fit. Oddly enough, the nipple shields that I used in the beginning while breastfeeding, which are made by Medela, come in 20mm size, which fit perfectly. Many women I talk to who have small nipples struggle with breastfeeding and rely on pumping to feed their infants. It is extremely disappointing that the company that makes the best pumps (and I admit the actual pump is AMAZING), does not make flanges that will work for a demographic that so heavily relies on their products - those of us with small nipples! Please, please, please, make smaller flanges for your pumps. Do not recommend to women with small nipples.

Rated 2 out of 5 by from Small nipples + big breasts = bad fit I bought the 21 mm size hoping it would work. The tunnel is still pulling in the breast tissue, which is painful and the outer cup is littler than the other breast shield, this causes leakage down my ribs and the loss of milk for my son. There is also the issue of my size. As a plus mom the angles are inflexible and cause issues with pumping. If the breast shields can in various nipple tunnel and flexible breast cups or an inder of some type that allows for the adaptation or adjustment of the nipple tunnel, I would give this 10 stars.

Rated 1 out of 5 by from Don’t fit waste of money I was super excited to start pumping I bought extra parts including this breast shield. I bought it in a 21 because one breast is a 21 and the other is a 24 and I have a ton of 24. I cleaned the parts and started pumping and let me tell you my breast hurts so bad from this breast shield. It sucks I spent the money I did to not be able to use them. But they are very nice I will. Say just didn’t fit right and they hurt.

Rated 1 out of 5 by from Disappointed in flange sizing options I was initially very happy when I purchased my Medela Pump in style, excited to use a hospital grade pump to ensure my newborn was receiving my breastmilk after having latching issues. Needless to say, my opinion of Medela quickly changed when I realized the smallest flanges they have available is 21mm.. I am a 15mm. Pumping is quite uncomfortable and can get painful, even on the lowest setting; and ultimately my supply is dwindling, despite my constant pumping every 2 hours for 30-60 minutes! I wish I would have known this before purchasing, as I would have never wasted $500 on what’s supposed to be the ‘best brand out there’. Medela needs to start providing sizes starting at 15mm. I really don’t understand why Medela does not, when there are other brands that do provide the smaller sized flanges; there are many women who need the smaller ones! If you are a mother or mother to be who plans on pumping, check your nipple size first and don’t waste your money like I did. Medela obviously isn’t planning on making smaller flanges anytime soon, as I’ve read reviews dating back over a year from women saying the same as I; as well as Medela responses saying they are ‘working on it’. I am utterly disappointed with Medela.

They are supposed to fit around the tip of your nipple with just a little bit areola. I bought the smallest size they carry (21mm) and it's still pulling half my areola through the tube. Also they could be made of a softer silicone material instead of rock hard plastic. If you are hating the way your pump is pulling on your breast tissue, look into pumpin palms. They carry a much wider range of silicone shields that can be placed into the medela ones. I can't believe healthcare pushes breastfeeding onto moms and lactation consultants don't talk about breast pumps. Do they think most nursing moms don't work? Rated 1 out of 5 by from Too big With literally every review for this product stating it is too large, I am shocked that there are still not smaller versions available. I’m about to have my second child and had a lot of problems with my pump last time around. After the 21 mm shields failed, I ordered smaller shields through a third-party vendor in 19 mm and they were still too big. There is obviously a market for smaller shields, why aren’t you taking advantage of this? If you were to offer a 19 mm, 17 mm, and even smaller, there would be a lot of grateful women out there including myself. It is worth noting that most women I assume are like myself and have no idea that their nipples are unusually small until they run into issues trying to breast pump.

Now I’ve paid over $200 for in item i cannot use without discomfort. I’m really displeased. And shocked the company doesn’t have more sizes !!

Less damage done to my skin than the 24mm but still had clogged ducts on both sides. Currently in the market for something different so I don't have to give up exclusively pumping.

Does this work with the Symphony? Or do I need to purchase the insert?

Hi Walter--- The 21mm Breast Shields are compatible with the PersonalFit connectors that are used with our Symphony Breast Pump. You want to ensure that you have the PersonalFit connectors that separate into two pieces and not the 1-piece breast shield and connector. If you have the 1-piece breast shield and connector that does not separate, then the 21mm insert would be recommended. The 21mm insert simply go inside the 24mm breast shield.
