I've got a 5-month-old at home and work 3 days a week for 12 hr shifts. I have thus far been able to exclusively feed him breast milk either via nursing or pumping and bottle feeding. I found out almost by mistake that I can increase the amount of milk I express while pumping if I compress the breast at the same time.

I have had some luck with this. A few things helped me that may help you:
1) are you in a quiet and relaxing place while pumping? I'm an RN and I work long 12-hr shifts. If I'm sitting in my office but thinking about everything happening with my patients, I don't have a milk ejection reflex.

If it seems that the shield size is correct, try using a different style. I find that the soft shields aren't as comfortable for me as the personal fit ones (those are the rigid plastic ones). It may be worth a try to buy and try a size up or down just for kicks to be doubly sure you have the right size for you.
