Participation Guidelines:
By voting on this site, you are helping us provide Medela products or services to four NICU facilities. The hospitals that are the top vote-getters from each of Medela's four regions will receive a donation worth at least $5,000 of Medela's Mothers' Milk Support System services or products.
Your participation in the Medela Virtual Human Milk Collection event ("Promotion") is subject to these Participation Guidelines, as well as our website Terms and Conditions, as well as all applicable federal, state and local laws.
PARTICIPANT CRITERIA: To participate in the voting process, you must: be a legal resident of one of the 50 United States or the District of Columbia who is 18 years or older and has reached the age of majority in their state of residence at the time of entry, and have Internet access. Sponsored by Medela, Inc., 1101 Corporate Dr., McHenry, IL 60050 ("Sponsor"). Employees of Sponsor or its affiliates, retailers, licensees, subsidiaries, advertising and promotional agencies, and the immediate family members of each, are not eligible to participate. Void where prohibited by law and subject to all applicable federal, state and local laws. Participation constitutes user's full and unconditional agreement and acceptance of these Participation Guidelines.
TO VOTE: To vote, visit the Medela web site at www.medelapreemieawareness.com ("Site"), and complete and submit the registration form. All information collected in connection with this Promotion is subject to Sponsor's Privacy Policy. Promotion begins on November 1, 2009 and all entries must be received by 11:59 p.m. CT on November 30, 2009. Incomplete entries may not be counted. All entries must conform to the voting standards and rules set forth herein. Also, all entries must be submitted Online.
AWARDS: The hospitals that are the top vote-getters from each of Medela's four regions will receive an award worth at least $5,000 worth of The Medela Mothers' Milk Support System services or products. Actual product/service donation will be determined by Sponsor based on need, but may include: Symphony® Preemie+™ program cards, Medela NICU Education programs, Symphony breastpumps, Waterless Milk Warmers™, Creamatocrit Plus™, Transport/Discharge Bags and/or BabyWeigh™ Scales. Certain restrictions apply. Recipients will be announced in December.
All donations will be awarded. Sponsor reserves the right to substitute a donation of equal or greater value if the specified donations is unavailable. All recipients will be notified by mail, telephone or email by December 10, 2009. Taxes are the sole responsibility of the donation recipient. All recipients must respond within ten days following attempted notification by Sponsor or an alternate recipient may be selected.
CONDITIONS: Each user may only vote for one NICU. Tallies will be adjusted daily after 5 p.m. Monday through Friday, except on Thanksgiving Day and the Friday following Thanksgiving. In the cases of multiple votes, fraud, or abuse, we reserve the right to eliminate votes cast.
By voting, participants agree to abide by the terms of these Participation Guidelines. Sponsor assumes no responsibility for, and each participant waives all claims in connection with, any lost, late or misdirected votes or for any problems of technical malfunction of any telephone network or lines, computer Online systems, servers, providers, computer equipment, software, failure of any e-mail or entry received by Sponsor on account of technical problems or traffic congestion Online, through the Internet, the Site or any combination thereof, including without limitation any injury or damage to entrant's or any other person's computer resulting from any cause, including without limitations downloading any materials related to the Promotion or Sponsor.
CAUTION: ANY ATTEMPT TO DAMAGE THE SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION MAY CONSTITUTE A VIOLATION OR CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES FROM ANY SUCH ENTRANTS TO THE FULLEST EXTENT PERMITTED BY LAW, INCLUDING CRIMINAL PROSECUTION.
Sponsor reserves the right to cancel the Promotion or change any element thereof, with or without notice at any time, due to infection by computer virus, bugs, tampering, unauthorized, intervention, fraud, technical failures, printing errors, or any other causes beyond the reasonable control of Sponsor which corrupts or affects the administration, security, fairness, integrity, or proper conduct of the Promotion. Sponsor also reserves the right to disqualify in its sole discretion any individual who tampers with the voting process, the Promotion, or who violates these Participation Guidelines.
